Tissue Viability/Leg Ulcer Service
The Southampton Tissue Viability Team are based at the Adelaide Health Centre and will see patients in the Community, Community Hospitals, GP Practice, Nursing Homes and in Clinic.
Our role is to support health care professionals in clinical practice by providing teaching, information and advice.
The service is for clients who have tissue viability needs and are registered with a GP in the Southampton area.

A practice nurse, GP or district nurse must complete this referral form and fax it to us.

Exclusion Criteria
- Patients under the age of 16 will require a supporting letter from parent and GP if self referring.
- Patients who self refer to be made aware team will contact HCP at GP surgery unless explicitly asked not to as a matter of courtesy.
- Any patient possibly will not be able to be seen by Service in clinic if unable to travel with own or hospital transport and/or be unable to transfer to couch (they will be offered Assessment at home, but not ongoing care)
Additional information
- All new patients receive a verbal and written appointment contact. Written information includes advice on length of appointment, location map, items to bring with them, appropriate footwear and clothing and pre-appointment questionnaire on their condition and how it has affected them.
- All patients receive a written health care advice leaflet where appropriate and ongoing contact details to Service and access times.
- Informal links with Podiatry to undertake joint clinic appointments
- Discharge is undertaken if the referrer requests only a one off Assessment to support care planning. Subsequent care is organised on an individulaised basis. When an episode of relevant care is completed - patient care returned to the referrer for leg ongoing support.
- All GP/referrer are sent formal discharge letter with any support/advice and onward planning as necessary within 48 hours of discharge.
- Patient and referrer have Office number (24hr answering service) to discuss issues
