Homeless Healthcare Team

A multi-disciplinary primary care team providing care to homeless people in Southampton.

Referral criteria

The team will register anyone who is homeless* in Southampton and is not appropriately registered with a local GP.

*Homeless includes people sleeping rough, living in hostels or other temporary accommodation, families in emergency accommodation or refuge houses for victims of domestic violence or human trafficking, and Gypsies and Travellers on illegal encampments.

Individuals can self-refer by presenting to the Two Saints Day Centre, 30 Cranbury Avenue in Southampton between 09:00 and 16:00 Monday to Friday. Alternatively, you can telephone us on 0300 123 3991 to book a registration appointment.


Referrals to the Mental Health team come via the GPs and Nursing staff at the Homeless Healthcare Team in the first instance


Families should initially be seen by their locality Health Visitor who can refer into the Homeless Healthcare Team health visitors if necessary.

GP surgery registration is open to anyone over the age of 18 who finds themselves without secure accommodation in the City of Southampton

Full service description

The Homeless Healthcare Team provides primary healthcare to registered patients. 

The Team also provides specialist Health Visting care to families who are homeless but who will be registered with a local GP surgery. 

The Team also provides specialist Mental Health care to people who are homeless and who may be registered with a local GP surgery.

Phone lines are open 08:30-16:30. *Between 16:30 and 18:30 Monday - Friday telephone calls will be directed to Solent GP Surgery.
