Referral criteria
Patients with a Southampton GP who are over 16 years old and have a confirmed neurological diagnosis or who have seen a Neurologist.
For patients registered with a New Forest, Test Valley and Southern Parishes GP, the service is available for 16 - 65 year olds with the exception of patients diagnosed with Motor Neurone Disease.
Full service description
The Community Neurological Rehabilitation Service specialists in the treatment of people with neurological conditions such as Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy, Stroke. The team consists of occupational therapists, physiotherapists, rehabilitation assistants, doctors, dietitians, speech & language therapists.
Following assessment in the home, the team will decide which treatments are most appropriate.
