Children's Community Asthma Service Portsmouth

The Community Children’s Asthma Service is provided by Solent NHS Trust and commissioned to provide care across Portsmouth CCG.-

 The service provides support and advice to children

  • With a diagnosis of Asthma
  • Who are registered with a Portsmouth GP

and may also have

  • Newly diagnosed Asthma
  • Interval symptoms despite escalating treatment
  • Poor compliance to treatment
  • Repeat oral steroids
  • Recent hospital admission
  • A home nebuliser
  • Repeated out of hours visits
  • Missing school or nursery due to asthma symptoms

Referral criteria

The referral route for this service is via health care professional

  • GP/ practice asthma Nurse
  • Post hospital admission
  • Hopsital consultant
  • Health visitor
  • School Nurse
  • Other Health Care Professional

Full service description

The service currently comprises of one full time Paediatric Asthma Nurse Specialist who is also a Nurse Prescriber.

The service works closely with GP ‘s and the hospital respiratory team.

The aims of the service are

  • To provide initial assessment and on-going telephone or face to face support as needed
  • To raise asthma awareness
  • Improve and standardise the management of Children’s asthma
  • Empower families to manage their child’s asthma at home
  • Reduce and Prevent hospital admission

Support offered to families

  • Families usually contacted within one month of referral
  • Given asthma management plan to help manage day to day and acute illness
  • Child can remain on a caseload and families can have on-going telephone or face to face support
  • Advice around Inhaler technique and compliance to treatment
  • School and nursery support and training
  • Medication explained and prescribed
  • Close links with GP and PHT
