Children in Care Health Team

Children in Care carry out a full health assessment on all children entering the care system. The service identifies any unmet physical, mental, emotional or sexual health needs in the child or young person and refers to the appropriate services. Children in Care provide ongoing support to social workers, foster carers and residential staff.

Referral criteria

Children placed at home under care orders. Care leavers; Children with a care plan for adoption; Children in foster care and residential provision; All infants, children and young people who become looked after by Southampton local authority up to the age of 18 years or 19 years where there are severe learning disabilities. This will include: 

  • Children in foster care and residential provision;
  • Children with a care plan for adoption;
  • Care leavers;
  • Children placed at home under care orders.

Initial referral is made by Social Services on entry to the care system.
Once the child is in care, referrals can be made by the young person, foster carers, residential staff, health professionals and/or education.

We can accept referrals for children or young people placed in Southampton by other authorities, provided funding is agreed in advance by the requesting health team.
