Looked After Children

The Solent East Looked after children’s service is a team of Community Paediatricians, and Specialist Nurses who are responsible for all children in the care of Portsmouth Local Authority aged 0-18. We work in close partnership with children, young people, carers and local authorities to ensure that children and young people’s health needs are supported whilst they remain in care until they become care leavers.

Referral Criteria

What we provide?
 
Initial Medical assessments:  Completed by Paediatricians for all children who are new into care.
Review health assessment: Depending upon age, this takes place six or 12 months after an Initial Medical and will be completed by a Specialist Nurse from the Looked after Children’s Team who ensure that high quality health assessments are offered to all Children who are looked after.
We offer health promotion, information and support around a wide range of health and lifestyle issues to support children and young people being able to reach their full potential
We provide Health Assessments to children who are placed in Portsmouth by other Local Authorities
