Children's Community Nursing (CCN) Service Portsmouth

The Children’s Integrated Community Nursing Service is provided by Solent NHS Trust and commissioned to provide care across 3 CCG’s - Portsmouth, Fareham & Gosport and SE Hampshire.

The service provides seamless care to children with a nursing need, including those who have

  • Complex health needs,
  • Chronic illness or conditions,
  • Life limiting/threatening conditions,
  • Palliative care needs
  • End of life care needs
  • Short term care – such as Wound care & IV antibiotics
  • Urgent care (COAST) to support acutely unwell children with one of the 10 specific conditions to be cared for in their own homes.

Referral criteria

Generic CCN 0 – 19years with a health need requiring nursing input that are registered with a GP covered by the CCG.

Urgent care (COAST) 0 -16yrs who are registered with a GP covered by the CCG. And have been assessed by a GP/Nurse Practitioner or hospital doctor and requires on-going review and assessment with one of the following 10 conditions:

  • Gastroenteritis 
  • Febrile Convulsion
  • UTI   
  • Asthma
  • Viral URTI 
  • Chest Infection
  • Tonsillitis
  • Croup
  • Viral Wheeze
  • Bronchiolitis

Full service description

The Children’s Community Nursing Service is provided by Solent NHS Trust and commissioned to provide care across 3 CCG’s - Portsmouth, Fareham & Gosport and SE Hampshire.

The service has a vision to provide seamless care to children with a nursing need, including those who have

  • Complex health needs,
  • Chronic illness or conditions,
  • Life limiting/threatening conditions,
  • Palliative care needs
  • End of life care needs
  • Short term care – such as Wound care & IV antibiotics
  • Urgent care (COAST) to support acutely unwell children with one of the 10 specific conditions to be cared for in their own homes.

The aim of the service is to provide a high quality in reach and outreach nursing service and to enable early, safe planned discharge from hospital and to prevent unnecessary admissions. These services are a link between primary and secondary care and support and encourage the development of partnership working.

The CCN service works in partnership with Acute and Community Paediatricians, , GP’s, Therapists, Health Visitors, School Nurses, Schools, Children’s Services and voluntary organisations to help the children and young people achieve their full potential and live as full a life as possible.

This care is delivered in the community location that is the most appropriate for that child or young person e.g. home, school, community clinic or respite centre.

The CCN team also provides nursing support into 3 Special Schools, Mary Rose Academy in Portsmouth, St Francis School in Fareham and Rachel Maddox School in South East Hampshire.

Generic CCN – By completing a written referral form and emailing this to the generic account

Urgent care(COAST) requires 2 steps to complete referral

  1. Verbal referral – Phone COAST nurses directly to discuss suitability of referral and capacity
  2. Email referral to Team account
