Jubilee House

Jubilee House Jubilee House is an inpatient unit situated in Cosham, comprising of 25 beds in total. The unit admits adults 18 years and older who are registered with a Portsmouth GP. The unit provides two roles:- 1. To carry out assessments for patients who are deemed to require Continuing Healthcare need assessment to ascertain what the long term needs of the patient are. As part of this process patients currently receive a Physiotherapy and Occupational Therapy Review to ensure that no opportunity is missed to maximise their independence. A multidisciplinary team comprising Nurses, a Physiotherapist and an Occupational Therapist work together, in collaboration with GP and social work colleagues who in-reach into the unit, to plan the patient’s discharge to the most appropriate environment with whatever support is needed. 2. To provide End of Life Care for patients who in the last stages of life. The Community Specialist Palliative Care Team are co-located in the same building which enables this inpatient unit to work closely with that team. The service operates in a collaborative and co-ordinated way across primary, secondary and community health services and social care to ensure timely and smooth transfers of care, to reduce delays within the system and ensure service users receive appropriate care in the appropriate setting. Nursing care offered is Specialist and provides high quality End of Life Care and Palliative Care

Referral Criteria

 

  • Patients will usually be over 18 years of age,
  • Patients will be registered with a GP practice in Portsmouth City CCG area.
  • Patients will be accepted from other CCG’s following written agreement from the relevant CCG Commissioner.
  • Patients have been assessed as requiring a  Continuing Health Care assessment and are at end of life.

The unit is open 24 hours per day, 7 days per week, 365 days per year.

  • Patients can be referred from the acute hospital or by GPs.
  • Prior to admissions, patients will have a Continuing Care assessment checklist completed, which will indicate whether there is a need for a full Continuing Care assessment.
  • The checklist must be faxed to Jubilee House and the patient placed on the waiting list.
  • Patients meeting ‘fast track’ criteria for end of life care will have end of life transfer sheet completed
  • If there is a waiting list for admission, priority will be given to patients who are end of life and patients in the Community who are otherwise at risk of admission to hospital..
  • When patients are admitted from the Community, GP’s will be requested to book appropriate transport for transfer to the unit.
  • GP’s are required to provide written information regarding their patient’s diagnosis and medical needs.
  •  Patients and Carers will receive information about the unit and criteria prior to admission.

           Exclusion Criteria

Children (under 16 years).

           Response Time & Detail and Prioritisation

  • Referral paperwork can be received at any point in the day/night.
  • Admissions from QAH can be accepted providing a bed is available, death is not likely to be within a working day of admission and full copies of medical notes and original drug charts are received
  • Patients can be fast tracked (admitted within 24 hours) provided there is an empty bed
  • Patients from the community are admitted as a priority over those already in hospital;   end of life patients would be the next priority.

Full service description

Jubilee House provides care to people suffering from multiple and complex health and age related problems, which require a complex assessment and care provision. It provides 24 hour nursing care for patients  and a holistic interdisciplinary approach to continuing NHS health care assessment for older people. Medical care is provided by a GP practice on daily basis. Out of Hours medical support is provided by the Out of Hours Service for Portsmouth City.

 

The unit provides:

  • Assessment for continuing health care – to enable patients and carers to make decisions about long term health needs.
  • End of Life Care for patients who are assessed as in their last stages of life.

 

The unit has a minimum of two trained nurses on duty at any time. Registered nursing staff provide assessment of patients physical, mental and social care needs and provide appropriate plans of care to meet individual needs.
