You can click here to read a report published on the NHS England website. The report commissioned by NHS England, was written by Niche Patient Safety, and is an independent investigation into the care and treatment of a mental health service user (Mr C) in Solent NHS Trust.
The report looks at how the Trust responded to the serious incident which involved Mr C, and investigates the circumstances surrounding the incident.
Since this incident, which took place on May 11 2013, Solent NHS Trust has taken important steps to ensure that the recommendations and advice outlined in this report have been put into practice.
