The Better Care Fund is about joining up health and social care services so that they work together seamlessly for local people. More support in the community will mean that people will stay healthier and maintain their independence in their own homes - avoiding hospital stays.
Better Care joins together part of the local budget for NHS and Social Care services from April 2015. Each NHS Clinical Commissioning Group (CCG) and local authority Health and Wellbeing Board has jointly agreed a Better Care Fund plan and these plans have been approved by NHS England. There are plans for Portsmouth, Southampton and for Hampshire. Our partner websites will be updated with news about Better Care and how the public and patients can be involved.
Solent NHS Trust will work more closely than ever with primary care and adult social care. Part of the plan includes moves to sit side by side as part of the same team, which will help avoid duplication and co-ordinate care around the needs of individuals and their carers. We are working closely with all of our partners to develop the plans. Residents should experience a service that is more focused on preventing their needs escalating.
The focus will initially be around services often used by older people and vulnerable adults. In future years it is planned that the Better Care Fund will be used to develop services for other groups, such as children and young people.
